Accessing HMS services through My Aged Care requires contacting your plan manager or support coordinator and requesting them to connect with HMS Community. If you do not currently have My Aged Care or a Home Care Package, please use the link provided below for assistance.

My Aged Care Info

It's important to understand that while HMS Community is dedicated to providing support once you are connected with My Aged Care or have a Home Care Package, we are unable to assist with the initial process of enrollment. Our focus is on your health and well-being, offering a range of aged care services tailored to your needs, including personal care and palliative care. Our team comprises of various professionals supported by a medical team of Community Paramedics & Nurse Practitioners.

NDIS

At HMS Community, we take pride in our highly qualified support workers, many of whom are healthcare professionals such as paramedics and nurses. Their expertise extends far beyond traditional care services, making them stand out from other providers. We are committed to helping our people achieve greater independence by offering a comprehensive range of support services to our NDIS participants:

From assisting with attending social outings and engaging in community activities to providing personal care, social planning, and therapeutic companionship, physical and emotional assistance, medication checks and administration, and collaboration with your family and other essential service providers.

Our paramedics and health providers are uniquely equipped to work with individuals facing high-risk situations, such as airway issues, seizure risk, or complex medical conditions, ensuring your safety and well-being. Additionally, we offer mental health support, health checks, advocacy during GP or medical appointments, as well as safety plans and strategies for managing potential hazards.
