Welcome to our HMS Community Health Hub, a dedicated space designed to support the health and well-being of our community.

Located at 2/18 Brantome Street, Gisborne, our hub brings together a diverse team of health and allied health professionals committed to providing accessible, high-quality care.

Our HMS Partners services include Physiotherapy, Chinese Medicine - Acupuncture & Herbalism, Nurse Practitioner Acupuncture, Play-based Therapy and Counseling, alongside the HMS Specialist Clinic (see below).

This hub is our way of continuing to keep our People and Patients safe in their homes and communities, providing holistic, person-centered care where it’s needed most.

Discover the HMS Community Specialist Clinic, located in Gisborne, open on Wednesdays (9am - 1pm).

Our expert team of Community Paramedics and Nurse Practitioners work collaboratively using a multidisciplinary approach to provide comprehensive assessment and management for people with ongoing and complex health needs.

Embracing a holistic approach, we provide personalised treatment plans for individuals with whom traditional treatments have fallen short.

Referrals are welcome but are not essential. Where appropriate, our team may liaise with your existing healthcare providers to ensure safe and coordinated care.

What is Managing Anxiety, Depression, PTSD and Sleep Program?
It is a 10-session, mental health-informed recovery program, usually delivered over 10 weeks by accredited clinicians. It’s designed to compliment your current care and support you in taking practical steps toward wellbeing.

The program includes:

  • Gentle exercise or movement
  • Therapeutic listening and conversation
  • Motivating tasks to reconnect
  • Tracking your progress
  • Planning your next steps

Who is it for?
This Program is for anyone experiencing anxiety, depression, PTSD, or sleep challenges. It’s especially helpful if you’re feeling isolated, overwhelmed, or unsure where to start.

With more than 20 years of experience in healthcare, I have worked extensively in Intensive Care and Emergency Department settings, where precise, timely clinical decision-making is essential. These experiences have given me a deep understanding of the strengths of Western medicine, as well as its limitations in addressing long-term, whole-person health needs, particularly for those living with chronic conditions, pain, women’s health concerns, and musculoskeletal issues.

To broaden the support I can offer, I completed training in Traditional Chinese Medicine (TCM) acupuncture at RMIT University Melbourne. My practice integrates this approach with my clinical background as a Nurse Practitioner, combining evidence-based medicine with holistic care. This allows me to provide a balanced, comprehensive path toward improving health, function, and overall wellbeing.

Dr. Anita Ackers is a dedicated Chinese medicine practitioner and lifelong member of Gisborne’s local community. A graduate of a Bachelor of Health Science and a Bachelor of Applied Science, Anita combines her academic expertise with a deep commitment to holistic care.

Anita enjoys working with dermatological conditions, paediatric support and providing effective solutions for pain management. She also has a particular interest in addressing gynaecological and mental health concerns, offering tailored support for hormonal health, fertility, and emotional well-being.

Aurélie is an experienced physiotherapist specialised in manual therapy based on the Sohier concept (analytical physiotherapy).

She graduated in France in 2005 and completed her Australian registration in 2012. Over time, she has developed expertise across both countries and has a particular interest in treating musculoskeletal conditions for patients of all ages—from children to elders. Aurélie tailors her treatments to help patients achieve swift recovery, autonomy, and long-term well-being. She is a strong advocate of the connection between physical and mental health, believing that "movement is life."

In 2014, Aurélie founded Synergy Physiotherapy and has successfully managed several physiotherapists since. She is committed to teaching the Sohier concept in Australia, regularly attending AIAPT conferences and local workshops to exchange knowledge.

Outside of work, Aurélie is a dedicated mother of two and enjoys participating in local sports clubs. She also finds relaxation tending to her organic vegetable garden.

Tracey is available every second Friday at HMS Community Health Hub in Gisborne.

Hi, my name is Tracey. I am a Social Worker with qualifications in counselling and community services. My working background is in Child Safety and administration within psychological and special needs sectors. I have a passion for ensuring children are given the opportunities to live their best lives and that parents have the tools to facilitate this. I believe in a holistic approach to working with people and truly believe that everyone can have a role to play in the therapeutic process.

With a Certificate in Art Therapy Practice, I am equipped to incorporate art therapy techniques into our existing play-based therapy approach.

Over the past year I have enjoyed watching my son learn and grow and move through his milestones and bring with me the knowledge gained from parenthood and its many ups and downs. This experience has heightened my empathy and understanding when it comes to parents and any concerns they may have about their wellbeing or development.

This means that when appropriate I incorporate techniques based on Cognitive Behaviour Therapy (CBT), Emotional Freedom Technique (EFT) (tapping), Acceptance Commitment Therapy (ACT), Art Therapy and other therapeutic and counselling techniques.

I work with people aged 15 and above and advocate for diversity, equality and inclusion.
